Lots of potholes last 1.5miles. One small section of ruts that are kind of avoidable but would be awful if muddy. I went slow and did okay without adding any more of natures pinstriping to my vehicle (I think)
No signal on Verizon, ATT, or T-Mobile or US cellular.
Woke up once in the night hearing people walk all around my camper. Got up and turned on the outside lights and scared the hell out of a herd of deer.
Vault toilet and trash can at trailhead. A few pullouts on the route for shorter vehicles. Maybe one or two my tow vehicle and 23ft trailer could have fit on without going into the road. I just camped at the trailhead and wasn’t bothered.

The road was well maintained until you come to a slight right turn, then there were large potholes all over, but most were shallow and it wasn't bad to drive over them, we are a large van/camper and had no issues. The GPS coordinates take you to a pull off with 2 camp sites, a little further up at the end of the road there is one additional site (be sure to leave room for someone to turn around), we stayed here it was our own nook to the forest with an overgrown old trail/road that we took for about a mile.

Went to scope out this location after calling the rangers office. I have a 34' class A and she said it might be a good spot. The road was extremely rough with lots of pretty deep holes and overgrown and downed trees that scraped the top and sides of my car. There is no way an RV would get down here. I doubt the camper in the pic would be able to navigate this with the current conditions. The area that appeared to be the camp sites had a lot of trash. But the actual trail to Fall Creek Falls was great. And well worth the time for this recon mission.

Spent the night here after doing the nearby Falls Creek Falls hike. A short trip up a good gravel road brings you to a hunter's camp with room for several vehicles. Stayed here in late August and never saw another vehicle.
