Visiting Theodore Roosevelt NP, but Cottonwood Campground in the NP was fully booked, and we were put off by the crowdedness and parking lot feel of the private Medora Campground (plus the price!!!!) so we decided to try our luck on a Saturday night and then again on a Monday night at this first come, first served campground just 7 miles from Medora west on I-94. Arriving around sunset the first night, we were delighted to find a handful of sites still available on a Saturday evening. We came back again on Monday night after returning from a day and night up at the North unit of TRNP and were delighted again that many sites were available after dark.
SItes are spacious and include a fire ring and large picnic table. (Both picnic tables were a bit strange, with a standard sized bench on one side and then an oversized bench on the other side and longer table.) We had a great short hike up to the highest viewing point for a sunset on Saturday night. Bathrooms were clean - looks like they're cleaned daily - and the showers were great: warm water and clean, and I think there was even heated air in the shower for getting dressed afterward.
Did not notice any noise at all from the Interstate, and we had our windows open all night. Fellow campers were quiet. My son enjoyed riding his bike through the paved campground loops.
We didn't try the dump station but they do have a good potable water station. Paid $20 cash each night in envelopes at the fee paying station.
Got one bar of AT&T, but it would intermittenly go to no bars and SOS on my iPhone. No t-Mobile access. Several sites without tree cover would have been fine for Starlink, but most sites have ample shade and room.

This park being FCFS meant we had to have a plan B, so it was either here or the next rest area, but fortunately one spot was open that worked for us. Some are reserveable but they were all taken, of course. We had to park the truck in the picnic/dump/pit toilet lot, but we were close enough we could see it from our site. The other sites were smaller, and even the couple that might have held the 45' fifth wheel would have been impossible to get into because the road is so narrow there's no turning room. We stayed 3 nights to take a breather from a 2,800-mile long haul and it was a welcome respite. The host kept the bathrooms and showers clean, and the whole park was very nice, and oh so quiet at night. Minor freeway hum didn't bother us at all, but site 37 has some distance and vegetation buffer compared to other sites. I had enough cell service to do research and book tickets for the show and fondue dinner, but didn't take notes. It's either AT&T or T-Mobile. We'd love to return.

We had a cool back in site that was surrounded by foliage, but we could see & hear the highway noise (didn't bother us). Many of the sites were sloped and would be better suited for tents, however, there are several sites with signs that indicate they are for RVs and ask the tent campers to select another site - very cool! Close to TR National Park and the right price. Worth a walk up the little hill for the view! Campground info video: https://youtu.be/ImN7Xt2mK1w

It's convenient to Medora, has showers, and the sites are well-spaced with good privacy. The two loops offer views in different directions and you can usually find shade if it's hot. I do believe it accepted our America the Beautiful pass, so we only paid $7.50/night in 2021. We would definitely return!

Overall: TLDR
Easy access from hwy, to Medora and TRNP. Plenty of sites. Will need to level. Large concrete pads. Clean.
Access:
Pretty close off the hwy. Some sites were quite unlevel. We drove through and ended up on Loop B in the back right next to the lookout hill. It seemed much better than most but still needed blocks. Many sites open when we arrived. Stayed 5/29/22-5/31/22
Location:
Easy drive to Medora and TRNP. Advise going to the TRNP entrance further away. We had issues getting through from the Medora side. Some hiking at site but we used it as a jumping off point for nearby sights.
Cleanliness:
We didn't use the bathrooms, we use our own. Didn't see any garbage. There was roadkill on one of our drives in and it was removed by the next time we walked by, maybe 30 mins - 1 hour.
Site Quality:
Concrete pad but not level. Very well maintained sites.
Noise:
Could hear semi's on road, couldn't hear cars while inside. Wind noise.
